If you are a new patient to our office, please visit our Patient Forms page to download our new patient forms that will need to be filled out when you arrive at our office. Printing them, filling them out and bringing them with you will allow us to attend to your medical needs more quickly than completing them upon your arrival. Insurances not accepted: BlueCross/BlueShield HMO, Molina
Medicare and some insurance companies do not reimburse for routine medical foot care such as trimming nails, corns and calluses. This fee is collected the same day of service.
Payment of deductible, coinsurance, copay, and non covered services is the responsibility of the patient or responsible party. All copays are due at time of service. The patient's account must be paid in full within ninety days. Please contact us for payment arrangements or other questions regarding the account.
If the account is turned over to a collection agency, all fees associated will be the responsibility of the patient or responsible party.
